Interest Application

Thank you for your interest in the HOME Program.  If you would like to enroll your student during our next open enrollment period, please fill out this form.  We are currently enrolling students for the 26-27 school year. A tour of our program and meeting with the Program Facilitator is required prior to registration.  We prioritize registering in district students before choice transfer students.  We will reach out to families on our waitlist to schedule a tour and meeting with the Facilitator as space is available.
